You must have excellent organisational skills, effective communication and interpersonal skills. Must be a team player and have a genuine interest in working in the community.Previous community experience would be an advantage as would phlebotomy skills. Patients are seen within own homes, and care homes in order to deliver excellent nursing care.The main focus of the team is preventing unnecessary admission, supporting earliest possible discharge from hospital and enabling patients to remain as independent as possible in their homes.
