What you need – Role requirements

An exciting opportunity has arisen for a Occupational Therapist to join STSS (short term support services) , STSS provides a single point of access to community intermediate care services, working with referring professionals to progress their initial assessment and provide the relevant services to the individual. Through collaborative working and holistic screening the team deploy and commission appropriate intermediate care services to ensure service users’ needs are met. The role will also involve holistic face to face assessments, reviews and support planning to support the transition onto appropriate services, please note that all assessments will be undertaken outside of the hospital setting.
