Clinical Care Coordinator

The role of the Clinical Care Coordinator is to provide clinical assessment to all Home Care Participants including risk assessments, Support Plans, WHS, Behavioural Management.

KEY ROLES AND RESPONSBILITIES
• Conduct clinical assessment for clients
• Upkeep of Incident Register, Continued Improvement Register and Risk Register
• Assist with NDIS and Aged Care Audits
• Provide clinical leadership and advice to management team and rostering team of Participants/Clients clinical needs
• Regularly review Participant/client care plans to ensure compliance to standards of care for quality review
• Monitor, evaluate and review care plans as required by client care needs and inform Management of any changes in the Participant/clients health
• Record and monitor significant health conditions
• Meet clients' clinical care needs under wound care, behaviour, pain and falls. Record and Manage these incidences.
