The purpose of this role is toplay a crucial part in supporting customerstransitioning from homelessness to Housing First long-term accommodation by fostering communityconnections, promoting social integration through education, training and employment.

What You Will Do
• Work within the framework of the overall objectives of the Housing First Service.
• Work within the overall policies and procedures of the Housing First Service.
• Engage with customers to identify their strengths, interests, and goals for community integration.
• Develop individualised community integration plans that align with customer’s goals and housing stability.
• Connect customers with local resources, including educational, vocational, and recreational opportunities to enhance social inclusion.
