About The Role

As aSocial Worker Accommodation Support, you will be responsible for:
• Providing a high-quality Social Work response to patients who are homeless or at risk of becoming homeless who present to Logan Hospital.
• Providing this response within the Emergency Department and across the inpatient wards of Logan Hospital.
• Delivering high quality evidenced based Social Work clinical services to patients/consumers within a highly functioning multidisciplinary team undertaking risk assessments of complex patients, clinical intervention, and discharge planning with a demanding caseload.
