Title: Primary Care Lead
Salary: $ 86,778- $102,091 - $114661 per year (pay range explained below)
Contract: Term- Until July 2025
Hours: Standard Hours - 37.5 hours per week
Location: Chilliwack, Matsqui, or Surrey
About The Role
The Lead, Primary Care, is responsible for leading collaborative establishment, coordination and implementation of primary care services, supports and initiatives with Fraser Salish First Nations communities such as the Joint Project Board initiatives and End-to-End Integration programs and services.
