An exciting opportunity has arisen for an experienced individual to work within the Integrated Hospital team Discharge Team. You will require to be a 1 Level Registered Nurse or AHP equivalent with post registration experience.

The post involves working within the framework of the Hospital Discharge Service, which aims to support capacity and flow by providing support and rehabilitation to patients to facilitate a safe and timely discharge from hospital.
