The NHS England ambition for Frailty Virtual Wards is to provide care closer to home to patients who would otherwise be admitted into hospital. With one of the highest older populations in the country and well-established integrated community services, the opportunity for the Frailty Virtual Ward (FVW) in Torbay and South Devon sits at this interface between existing community services and specialist services within the acute setting; supporting existing community teams including Intermediate Care and Primary Care to manage those patients that are more acutely unwell or complex preventing avoidable admission or expediting earlier discharge.
