As a Registered Nurse - Home Care (RN) reporting to the Home Care Manager, the role of the RN is based on the Alberta Health Services (AHS) strategic direction and the Continuing Case Management Competency Framework. Using a person-centered approach, the RN supports the client/family to identify needs, focus on strengths, and set/achieve goals across the continuum of services. The RN demonstrates stewardship in service allocation engaging community and client resources. Responsibilities include assessment of client needs, establishing goals for client care delivery, planning, delivering, and coordination of care needs, resources and services within professional practice standards and Home Care program guidelines, communication and coordination of client care with the interdisciplinary team, and evaluation of outcomes.
