Job summary

This is a new and innovative role within the PCN, and we are looking for an experienced ANP to join anestablishedCare Home Team of clinical and non-clinical staff to cover the nine care homes in the district. You would inform on its development and processes to enable the delivery of high quality planned and proactive care for care home patients and to avoid unnecessary hospital admissions.
Main duties of the job

• To work within the Care Home Team to provide patient-focused care.
• To work in partnership with the Primary Care Network to develop the role and team further.
• To support practices with ongoing management of care home patients and amending Personalised Care & Support Plans and RESPECT forms.
• To support care home patients with anticipatory care discussions and planning.
