Summary

The VA Sierra Nevada Health Care System (VASNHCS) Community Living Center (CLC) Registered Nurse (RN) is a professional nurse who provides direct care to Veterans with a variety of chronic medical conditions. The CLC is a 60 bed unit, and RNs care for Veterans with an array of conditions including short and long stay rehabilitation needs, SCI, dementia care respite care and hospice. The CLC RN provides 24/7 care in a homelike environment.

The CLC RN assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, and supporting factors that impact customer satisfaction.
