Responsibilities
• Conducting assessments of individuals' needs for social care and support, considering physical, emotional, and environmental factors.
• Developing and implementing care plans tailored to everyone's circumstances, promoting independence and well-being.
• Providing advice, guidance, and practical support to individuals and their families/carers regarding available services and resources.
• Liaising with healthcare professionals, community agencies, and other stakeholders to coordinate and deliver integrated care services.
• Safeguarding vulnerable adults by identifying and responding to instances of abuse, neglect, or exploitation, following established protocols and procedures.
• Advocating for clients' rights and interests within the social care system, ensuring they receive appropriate support and access to necessary resources.
