Responsible for the management and coordination of care management services across the continuum of care at Hannibal Regional Healthcare System. Responsible for the development and implementation of the organization#s care management plan. This position facilitates an effective transition planning that supports the Hannibal Regional Healthcare System#s model, quality patient care, patient safety and financial components; promoting integration of a seamless care model; coordinating patient throughput; healthcare team collaboration and coordination as evidenced by metrics, optimizing performance and adoption of best practice. Serves as an advocate for patients, assisting them to reach optimal function and health in an appropriate environment that supports their needs. Oversees social services program.
