This position is responsible for leadership and oversight for daily unit operations as designated by the Care Management Director. Participates in patient care activities both directly and indirectly as a primary resource to the outpatient care management team. Intended to follow the patient beyond the post-acute visit, coordinating care across the continuum. Providing RN Care Management services designed to improve individual and/or family/ significant other overall health and well-being to a specified/targeted patient group and their families. Using various assessments to identify actual needs and potential risks for adverse outcomes, the RN Care Manager coordinates comprehensive health care and community-based services, home visits and services to meet the needs and risks identified.
