About The Role
• You will provide individualised care and education for people living with type 1, type 2, and gestational diabetes in both outpatient and inpatient settings.
• This important role aims to improve outcomes for people with diabetes and other chronic diseases through the provision of assessment referral and clinical services.
• This role also educates and empowers health professionals, carers, and the general community to better manage diabetes.
