This is a part-time, working from home position Support Coordination position at The Journey Company. starting 2 days per week, must be able to work Mondays and Fridays.
- Interpreting and implementing NDIS plans in alignment with the participant's goals and needs, leveraging mainstream and community services.
- Leading multi-disciplinary teams to coordinate various services and supports, ensuring seamless delivery of participant plans.
- Identifying suitable services, engaging with diverse support providers, and facilitating progress towards established goals.
- Addressing crises, barriers, and risks comprehensively, devising effective solutions to mitigate these challenges.
- Cultivating robust collaborative relationships with government agencies, non-government organizations, service providers, and allied health professionals.
- Empowering participants to self-direct, self-advocate, and manage their NDIS plans autonomously, fostering independence and choice
