About the Position: The primary purpose of this position is to serve as a clinical case manager to provide timely, quality, comprehensive, cost effective healthcare resource utilization and support for our Department of Defense beneficiaries with complex care needs. This is accomplished through clinical case management, care coordination and discharge/disposition planning utilizing military and/or civilian (purchased care) health care systems and resources.
