As a Health and Wellbeing Coach, you will play a crucial role in empowering our clients to achieve optimal health and wellness by providing personalized coaching and support. Responsibilities:
• Use coaching skills to support people to make conscious and informed health choices, change behaviours and encourage proactive management and prevention of illness.
• Work closely with MDT colleagues, including link workers, to provide access to self-management education, peer support and social prescribing
