Single Point of Access are looking for an experienced nurse who is highly motivated and has excellent communication skills, to join our team based in Ellesmere Port. Working as a Nurse Coordinator, the post holder will effectively manage referrals from a variety of care settings and utilise various care pathways, ensuring that patients are referred to the most appropriate service for treatment. This is a non-patient facing role working within a call centre setting. The post is full time 37.5 hours per week. Hybrid working available. The post holder is required to be flexible as shifts are allocated on a rota basis.
