Department: Home Health Care FTE: 1.00 Full Time (30 - 40 hours per week) Shift: Days Sign on Bonus and Relocation Reimbursement for External Candidates. The Clinical Advancement Program (CAP) is additional pay to recognize Nursing Education and National Certifications and can add up to $8.00 in addition to base pay. Position Summary: Provide direct nursing care to patients and families outside the hospital setting. Coordinate home healthcare activities. Ensures adherence to Hospital and Departmental Policies and Procedures. Patient care assignment may include; Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.
