Qualifications & experience
We are seeking an experienced pharmacist to join our team -
Excellent verbal and written communication
A flexible approach
Confidence interacting with health professionals
Ability to work both independently and as an integral team member
Tasks & responsibilities
Medication reviews in consultation with the GP
Provide patient education about how medications work and why they have been prescribed
Clinics that focus on hypertension, gout, cardiovascular risk factors etc
Ask your pharmacist clinics
Enable smooth transitions of care from hospital to home
Medication reconciliation and clinical audits
Management of repeat prescribing requests
