We are seeking to recruit an experienced and motivated individual to the role of Advanced Practitioner to work within the HMR Rapid Response Team, providing urgent community response to the population of Heywood , Middleton and Rochdale.

We are a community based, multi-disciplinary team of skilled professionals, who provide holistic care and support to enable people to remain in their own home and avoid unnecessary admission to hospital.

This is an exciting opportunity to join a multidisciplinary team encompassing both health and social care alongside links with community, primary and secondary care.
