Role Description

This is a full-time remote role for an Outreach Support Worker. The Outreach Support Worker will be responsible for providing assistance and support to individuals and families in need. The role involves engaging with community members, conducting assessments, developing support plans, and connecting individuals to appropriate resources and services. The Outreach Support Worker will also provide emotional support, advocacy, and crisis intervention as needed.
