Assistant Practitioner, Band 4, working for the Health Visiting Service, undertaking delegated tasks by a Registered Practitioner / SCPHN HV.

The ABUHB, Monmouthshire Health Visiting Team are enthusiastic, motivated and forward thinking in delivering care to children and their families. You will be supported in your role by other professionals to become a valued member of the team.
Main duties of the job

An exciting opportunity has arisen for an Assistant Practitioner to work with the Health Visiting team in Monmouthshire, delivering services to Children and Young People providing a preventative service as identified in the needs assessment carried out by the Registered Practitioner.
