With more than 120 physicians across 19 sites within London/Middlesex, Oxford and Elgin counties, the Thames Valley Family Health Team is committed to providing comprehensive, collaborative primary health care to over 169,000 patients.

The mental health & addictions care manager is a core member of the collaborative care team, including the patient’s medical provider, primary care team, psychiatrist, and community team. The mental health & addictions care manager is responsible for supporting and coordinating the mental health and addictions care of patients on an assigned patient caseload. This role collaborates with the patient’s , primary care team, psychiatrist and community partners.
