An exciting opportunity has arisen for a highly motivated Registered Nurse to join our Community Nursing Team in the North Westof the county in the provision of Planned and Admission Avoidance care to patients in the community.

We are looking for a caring, motivated friendly community nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.

Ability to travel independently to fulfil the requirements of the post is essential.
