Job Description

An exciting opportunity has arisen for knowledgeable an experienced social work Team Leader to oversee and manage the Independence and Wellbeing Service (IWS) Community Facing Team in the East of Cornwall, a role in which you will be expected to guide and support the team to utilise evidence-based practice to deliver strengths based and holistic outcomes for residents.

The Service & Team:

Within the IWS, the team focus extends through from initial contact and assessment work to gather information to consider the most appropriate next steps; utilising a solution focused approach. This would include providing information and advice, responding to Safeguarding Concerns, signposting, connecting to other agencies, working closely with the voluntary sector, progressing to reablement or undertaking an assessment with a focus on maximising independence and wellbeing through utilising social care and occupational therapy pathways.
