This is an exciting opportunity for an enthusiastic and experienced Mental Health Practitioner to join our General Practice integrated mental health service (GPimhs) in Weybridge and Hersham PCN.

We have a Fixed Term (12 Months), Full-Time (37.5 Hrs per week, 9am-5pm, Monday-Friday with limited flexibility) role.

Should the post be offered to an internal (SABP) applicant, this can be offered as a secondment.

The post-holder will be required to work from the following GP Surgeries in the Weybridge and Hersham PCN and sometimes for nearby SABP Hub.
• Church Street Practice, Weybridge KT13 8DW
• Fort House Surgery, Walton-On-Thames KT12 1UX
• Hersham Surgery, Walton-On-Thames KT12 4HT
• Rowan Tree Practice, Weybridge KT13 8DW
