The job itselfCommunity Support Worker – ReablementThe community support service supports people to regain their independence through reablement. We are a CQC registered service that is rated overall ‘Outstanding’.Our reablement team works with individuals to increase their ability to look after themselves following illness or injury and to rebuild the skills and confidence to help them live as independently as possible. We are working closely with colleagues and partners in health to ensure that we have an integrated and collaborative approach to our service. We are different from traditional home care, as we support and encourage people to carry out everyday activities themselves. We work closely with our occupational therapy and social work colleagues to support people coming home from hospital and support people in the community to maintain their independence.You will play a critical role in identifying meaningful and achievable goals that matter to the person.
