Job summary

In partnership with Rayleigh and District Primary Care Network (PCN), GP Healthcare Alliance (a federation of GP practices in Castle Point and Rochford) is looking to recruit a full time care coordinator to join the team to provide a central co-ordination for patient care planning across our member practices. This role is full-time and from Monday-Friday.

The Care Coordinator will work closely with the multidisciplinary team, and member practices, to identify and manage a caseload of patients, making sure that appropriate support is made available to them and their carers and ensuring that their needs are addressed.
