Alberta Health Services Central Zone is looking for qualified candidates to fill the position for a Social Worker II within the Transition Services and Seniors Health Portfolio. The successful candidate will work both independently and with multi-disciplinary teams providing family and patient centered care within the acute care and community portfolio. The Social Worker provides psychosocial services using a collaborative approach to support clients, families, and health professionals across the continuum of care, including Acute Care, Home Care, Supportive Living and Long-Term Care. This support may include, but is not limited to, supportive counselling, resource-based interventions completing psychosocial assessments, comprehensive risk assessments, cognitive screening, supporting transitions and discharge planning.
