Job summary

An exciting opportunity has arisen for a Community Practitioner to join our existing Community Learning Disability Team in Northumberland. The role of the Community Nurse will work across 3 streams within the pathway; Physical Health, Mental Health and Positive Behavioural Support .

The post currently sits within the learning disability pathway provided by CNTW. Close partnership working and liaison with Social Workers and Care Manager's from the Local Authority and other partners connected to the pathway is another key component of the role, particularly as the service is moving quickly towards a joint Learning Disabilities Pathway.
Main duties of the job

The role of the Community Practitioner, is to complete Specialist Nursing Assessments, act as a CPA coordinator, devise and implement care plans and risk assessments to support people with learning disabilities to live as independently in the community leading to improvements in their quality of life.
