Job summary

An exciting opportunity has arisen in the Northumberland Children and Young Peoples Service. We are recruiting band 6 Community Practitioners to work into our Mental Health/Learning Disability Pathways.

Our service is fully integrated and provides specialist mental health assessment and intervention for children and young people with complex and enduring mental health problems, including those with a learning disability. The multi-disciplinary team comprises of a highly skilled and experienced workforce.

To be responsible for the assessment and management of casework of service users who have substantial and complex mental health needs.

To provide evidence based interventions following a full assessment as part of a multi-disciplinary team. To complete documentation within robust timescales, and ensure review.

To undertake duties in supporting families and partner agencies, and attend multi-agency meetings to safeguard children.
