Job Description

The Assessor Coordinator - Degree Senior is accountable for assisting the Home Care Manager to ensure efficient and effective coordination and delivery of home care services. This position ensures a high quality of client care in keeping with the philosophy, objective, policies, and procedures established by the HHR. The Assessor Coordinator - Degree Senior through a collaborative case management process is accountable to ensure that the client has access to appropriate, adequate, high quality, coordinated and integrated service that is continuous throughout the healthcare agencies (system.)
