Role Description
• Engage clients while they are going through treatment, explaining the benefits of the alumni program. Enroll the clients in Alumni Program and Social Media groups prior to discharge.
• Conduct routine wellness calls to all discharged patients, enrolled in the Alumni program or not, to confirm patient adherence to aftercare/recovery plans
