SASSE3PCN, comprising Hythe Medical Centre, St Davids Family Practice, StainesHealth Group and Stanwell Road Surgery are looking to recruit CareCo-ordinators to join our Practice team to cover maternity leave.

A patient care co-ordinator (PCC) isan exciting and developing role which involves dealing with patients and, ifappropriate, their carer, before or after the patients consultation with aclinician or other healthcare professional.

Working closely with the patient andtheir clinician or other healthcare professional, the PCC co-ordinates patientshealthcare and directs them to the appropriate service to ensure that they getthe most suitable care from whatever health or social care provider isappropriate.
